PLAYER COMMITMENT:
1-Year Term “Champions Code of Conduct and Team Commitment” contract:
- Mutually agreed Player Fee Payment Plan (see chart below)
- 5-STAR Champions Program Player Declaration
- Live the Champions of Character Values: Integrity, Respect, Responsibility, Sportsmanship and Servant Leadership;
- Earn and maintain a “B” average or higher in all classes every reporting period AND 97% Attendance at School;
- Completion of a Team or Academy Community Service Project by season deadline;
- Participate and engage in the USLA College-Career Player Pathway Program;
- Research, discover and seek guidance from USLA Leadership on individual self-value and lifelong commitment..
- Attend ALL trainings, games, tournaments and USLA events, meetings and orientations identified on USLA Master Schedule;
- NO MORE THAN: 3 excused absences for trainings, and 2 excused absences for games/tournaments during a season. All USLA events, meetings, and orientations are MANDATORY.
USLA COMMITMENT:

Towards this effort, USLA offers a financial scholarships program and various payment plan options. Financial Scholarships Applications Available during the registration period, for players meeting the following criteria:
- Provide proof of FREE OR REDUCED School Lunch program eligibility and/or student lunch school number for verification (50% Registration Fee Deduction);
- Provide a copy of your Federal Tax Return for 2015 showing household size and household family income. Verification for Free or Reduced lunch will be determined off the Federal Lunch Program Eligibility Chart for the 2016-2017 year based on household size and income. For example for a family of 4 the eligible household income is $44,000 or lower, per year (50% Registration Fee Deduction);
- For families with 3 or more siblings registered in a USLA program team, and meet the eligibility criteria set forth under 1 or 2 above, an additional $100 per child deduction may be applied;
- Severe financial hardships will be reviewed on a case-by-case basis and a Financial Hardship Payment Plan may be recommended by the DOA/DOC and approved by the USLA Board;
- For severe financial hardships, a Volunteer / Community Service Work Plan may be offered to players/parents as negotiated by the DOA/DOC and approved by the USLA Board, for player registration fee credit;
- For players ages 16 and older, a Work-Play internship will be coordinated with USLA Business Partners to cover the full Annual Cost / Fee per Player of $780 for the program year. Anything earned above the $780 may be retained by the player.
